An exciting opportunity has opened to join our team of Dementia Care Mappers
The Dementia Care Mapping Team works in partnership with key organisations across the City to raise understanding and awareness of dementia and to drive up standards of care for people with dementia and their carers by facilitating and encouraging services and carers to improve their skills to help maintain daily living skills and promote independence for people with Dementia.
Dementia Care Mapping ™ (DCM™) is a set of observational tools developed by Bradford University designed to evaluate quality of care from the perspective of the person living with dementia.
The main purpose of the post holder is to:
Be an ambassador for Dementia Care and be able to guide and support services to develop person centred practices.
Undertake Dementia Care Mapping and assessments of the environment which will enable you in providing expert advise to colleagues, partners and stakeholders across Adult Social Care.
You will work within various care settings such as residential homes, day centres, extra care and supported living settings to observe people with dementia over the course of a typical day and record, using set codes, their actions, behaviours and wellbeing.
You will use a variety of tools that will enable you in writing comprehensive reports that include recommendations and identify key objectives to achieving "excellence" in Dementia Care across the city.
You will support commissioned providers to understand and implement best practice in the delivery of DEMENTIA CARE within their services.
Previous experience within Adult Social Care settings is essential for this post.
Excellent communication and written skills are essential for this post.
