We are looking for an experienced practice nurse to join our busy practice located in Trowbridge, Wiltshire. You will be required to provide diabetic care to our patients and hold the appropriate qualification in the management of diabetes.
The successful candidate will ideally have at least 2 years of experience in primary care practice nursing with a strong ability to identify, manage, and support patients at risk of developing long term conditions & dealing with existing chronic disease management. You will need to be organized, self-motivated and able to prioritize the workload.
Lovemead Group Practice are a patient focused practice covering a population of 19,600 patients. We are looking for a strong team player to join our existing team. We have an excellent PCN multi-disciplinary team and we aim to create a positive patient experience.
