This post is based within the Nutrition & Dietetic team at Peterborough City Hospital, but may involve working across Trust sites at times according to service demand. There is easy access between sites by car or train.
The team are part of the Rehabilitation Services Department consisting of Dietitians, Speech & Language Therapists, Occupational Therapists & Physiotherapists, who are committed to joint working to deliver a high quality service.
To undertake assessments and treatments for adult patients that have been triaged and deemed appropriate for Dietetic Assistant Practitioner intervention, with case management advice available via indirect and direct contact with a Registered Dietitian.
To manage own caseload of non-complex patients assessing, according to competencies, and devising and implementing treatment programmes.
To work collaboratively within Multidisciplinary and Multi-agency teams with the aim of supporting timely discharge.
To enable patients, their families and carers in the attainment of their Nutrition goals by monitoring treatment programmes developed by Registered Dietitians.
To deliver safe, efficient and effective patient focussed care in line with local standards.
To be actively involved in the in-service training programme, and supporting students as required.
To assist hospital Dietitians to deliver a high standard of general dietetic care that is patient centred and develops in line with national and local policies and initiatives.
