Job description

The role of the transfer of care hub is to work alongside other organisations and agencies to facilitate the safe discharge of patients who require support following discharge. The hubs bring together: acute and community health partners; adult social care and reablement; and other partners including housing, neighbourhood teams and voluntary sector. Wherever possible, patients will be discharged home and where necessary, with care and support wrapped around them to maximise their independence and wellbeing.
The team provide a single point of access for ward staff and therapy teams and are contactable by email seven days a week from 08.30am-4.40pm.
As a Discharge Coordinator working with the Discharge Team at Lincoln County Hospital, you will be allocated to an area and will be responsible for assisting in the discharge planning for our complex patients. This is a patient facing role. You will liaise with the wider MDT, both internal and external, including adult care, community health, reablement services, therapists, and others, to plan a timely discharge.
The role can be extremely challenging and we are looking for persons who are enthusiastic, work smart to solve problems and are able to work independently as well as part of a team.
Once a pathway is identified the team then support the wards to complete referrals and signpost to the correct supporting services and co-ordinate activity to ensure the patient, family or those acting on behalf of the patients, experience as smooth a transition from hospital to home with support, or into another service. This co-ordinated approach to discharge reduces the workload of ward-based colleagues with fewer follow-ups for information and provides more timely discharges with better outcomes for patients, reduced length of stay and improved flow for patients in need of acute care.
Each ward is be visited daily by a discharge co-ordinator in the morning, the team aim to designate the same co-ordinator to aide communication and consistency and have, due to this, built up very good relations with the wards.
