Job description


Ward F2N Cardiac Surgery is a new 16 bedded cardiac surgery ward which will be working alongside Ward F6 in caring for patients undergoing both routine and complex cardiac and aortic surgery.
As an extension of our existing cardiac surgery service, we have a vacancy for a new Discharge Coordinator role on F2 Cardiac Surgery, to facilitate the safe effective discharge of our patients.
We are looking for someone with both excellent communication and IT skills to come and join out team and work alongside our Ward F6 Discharge Coordinator to optimise this service. This will include providing cover for annual leave to ensure the smooth running of the service.
The role involves both direct liason with patients identified for discharge following ward round, as well as clinical duties removing sutures and cannulas and checking wound sites, prior to discharge.
Effective communication skills to support liason with the multidisciplinary team as well as the trust discharge team, social services and OT is essential.
The role also involves patient education on VTE and cardiac wound care as well as completion of District Nurse referrals and referral and transfer to the Discharge Lounge.
IT skills are essential and familarity with the HIVE system would be an advantage.
In addition the successful candidate will join the trust Daily Discharge Teams meetings to provide the names of patients identified for discharge and highlight any potential barriers to discharge which need escalating.
