The Royal Marsden is a world leading Cancer Hospital specialising in diagnosis treatment care education and research. It plays a vital role in pioneering and improving cancer care.
We are seeking a highly motivated and compassionate individual to join our team as a Band 4 Discharge Navigator. As a Discharge Navigator, you will play a vital role in ensuring smooth and efficient transitions for patients from hospital to their preferred place of care or living arrangement.

The post holder will work as part of the Complex Discharge Teamto assist the Discharge Coordinators to facilitate a timely, effective, and safe discharge of patients from the acute setting to the community. The post holder will be expected to work with NHS patients and Private Care patients as required in order to achieve best outcomes for patients, assisting in reducing length of stay.
The Discharge Navigator will assist the Discharge Coordinators by giving support and advice and acting as a resource for all aspects of discharge planning. Their role will be to assist the Discharge Coordinators and ward staff in complete referral documentation that accurately reflects the patient’s needs on discharge.
Excellent communication and collaboration with colleagues working in hospitals, community, and social and health services is required in order to promote a seamless discharge/transfer for all patients.

• The post holder will work in partnership with the multidisciplinary team to educate patients/families to assist and enable them to make informed choices in the planning of the discharge.
• The post holder will liaise where appropriate with Pharmacy to minimise delays in patients receiving pharmacy prescriptions to take away (“TTA”).
• Signpost patient and relatives concerns or complaints regarding discharge issues to the Discharge Coordinator/discharge Lead, utilising as necessary PALS or other appropriate agencies.
• The post holder will assist the Discharge Coordinator along with the palliative care specialist team, in the discharge planning arrangements for patients receiving end of life care, ensuring that all necessary paperwork, referrals are sent and received by community services to facilitate rapid discharge from hospital in line with national guidance and best practice.
