We are looking for a full-time Care Coordinator to work with an esteemed and well-established care company, based in Birmingham.
The working hours for this role are from 8:30 to 18:00, Monday to Friday; as well as being on call for weekends to cover any eventualities that may arise.
Day to day, you will need to :
- Carry out risk assessments, and maintain a high level of confidentiality and respect for patients
- Speak to social services and the hospital discharge team
- Work as part of a team
- Prepare reports related to the progress of patients
- Possess excellent interpersonal skills and the ability to interact well with different kinds of people
- Hold excellent verbal and written communication skills and the ability to prepare comprehensive reports
- Have a passion for helping people and providing personalised care
- Have strong leadership and management skills, and hence the ability to motivate others
- Possess an in-depth understanding of the organisation’s policies and medical regulations
- Be proficient with computer usage, particularly using Microsoft Office tools such as Word, PowerPoint, and Excel
- Hold strong analytical skills and the ability to handle multiple tasks at a time
