Domiciliary Care Co-Ordinator

We are looking for a full-time Care Coordinator to work with an esteemed and well-established care company, based in Birmingham.

The working hours for this role are from 8:30 to 18:00, Monday to Friday; as well as being on call for weekends to cover any eventualities that may arise.

Day to day, you will need to :

1. Carry out risk assessments, and maintain a high level of confidentiality and respect for patients
2. Speak to social services and the hospital discharge team
3. Work as part of a team
4. Prepare reports related to the progress of patients
5. Possess excellent interpersonal skills and the ability to interact well with different kinds of people
6. Hold excellent verbal and written communication skills and the ability to prepare comprehensive reports
7. Have a passion for helping people and providing personalised care
8. Have strong leadership and management skills, and hence the ability to motivate others
9. Possess an in-depth understanding of the organisation’s policies and medical regulations
10. Be proficient with computer usage, particularly using Microsoft Office tools such as Word, PowerPoint, and Excel
11. Hold strong analytical skills and the ability to handle multiple tasks at a time
