Purpose of the post

•Ensure that the people of Gwynedd are at the heart of everything we do.

•Assist individuals with learning disabilities to live their lives in the way they wish to live, assist them to develop skills and ensure opportunities to be a part of their community

Responsibility for functions

•Ensure appropriate use of specialist equipment including reporting defects and following specialist practice and training protocols as required

•Ensure appropriate use of individuals' property e.g. medicine and money

Main duties

MAIN DUTIES AND KEY TASKS:

SUPPORT THE INDIVIDUALS WHO USE THE SERVICE

•Work to meet objectives of the individual you are supporting, and ensure you respect the individual’s rights.

•Follow an ActiveSupport ethos (for more information click here: Active support) and Positive Behaviour Support (for more information click here: Positive Behaviour Support), that emphasise the importance of working with individuals and promote their independence.

•Assist with personal care in a dignified and respected manner.

•Contribute to/follow the individuals Personal Care Plans.

•COMMUNICATION AND RELATIONSHIPS

•Co-operate and communicate with professional workers and families/carers.

GUIDANCE AND POLICIES

•Act in accordance with the requirements of the Social Services and Well-being Act 2014 and the service and Gwynedd Council’s guidelines and policies (For more information click here: Social Services and Well-being Act Wales).

•Commit to a Professional Social Care Code of Practice (for more information click here: Code of Practice).

TRAINING AND SUPERVISION

•Take advantage of the opportunity to receive supervision and mentoring from experienced staff members.

•Evaluate and follow Gwynedd Council's Learning and Improvement programmes that are mandatory for professional development.

•Committed to completing professional qualifications when needed.

SAFEGUARDING

•Mentor new staff in their development

•Qualified to be able to assess and order equipment for individuals

•Responsibility for administering, distributing and providing medication in accordance with policy

•Can undertake simple health tasks

•Carry out a detailed assessment and identify changes in health and well-being

•Follow local and national policies and procedures to identify, record, report and participate in safeguarding vulnerable children or adult protocols.

•Responsibility for self-development.

•Ensure compliance with Health and Safety rules in the workplace in accordance with the responsibilities noted in the Health and Safety at Work Act 1974 and the Council’s Health and Safety Policy.

•Operate within the Council’s policies in relation to equal opportunities and equality.

•Responsible for managing information in accordance with the Council’s information management guidelines. Ensure that personal information is treated in accordance with Data Protection legislation.

•Commitment to reducing the Council’s carbon emissions in accordance with the Carbon Management Plan, and to encourage others to act positively towards reducing the Council’s Carbon Footprint.

•Undertake any other reasonable duty which corresponds to the salary level and responsibility level of the job.

•Responsibility to report any concern or suspicion that a child or vulnerable adult is being abused.

Special circumstances

•The post-holder must be able to undertake manual handling tasks which can include wheelchairs and hoists.

•This job description is only a general guide. Perhaps all Support Workers will be required to undertake duties that have not been outlined above at the Team Manager or the line manager's request.

•Expected to work across other sites of the service if mandatory.
