I am supporting GMC in their search for a temp on-going Early Help Practitioner
We are seeking a dedicated and passionate individual to join our team. In this role, you will be responsible for a variety of tasks aimed at supporting families and promoting their welfare.
Main Duties and Responsibilities:
- Manage a caseload of families.
- Undertake whole family assessments using evidence-based tools.
- Work directly with parents, carers, children, and young people, ensuring good outcomes are achieved.
- Co-produce plans with families and other agencies that contain SMART outcome-focused goals.
- Lead or participate in formal multi-agency reviews in line with practice standards.
- Deliver practical interventions using evidence-based tools.
- Complete accurate recording on case files, evidencing the voice of the child throughout intervention and planning.
- Work in a whole family way, promoting and developing parenting skills.
- Organise and lead group work with parents, young people, and children.
- Ensure children and their parents have the opportunity to contribute to the development of the service.
- Work with education partners and other key stakeholders to identify threshold appropriate support and intervention.
- Support the development of the Family Hub model.
- Effectively utilise supervision to discuss casework, training, or any other issues as appropriate.
- Ensure adherence to council policies and guidelines.
- Contribute to team meetings and the planning process.
- Participate in appropriate training and development programmes.
Qualifications:
- Experience in a similar role is preferred.
- Strong interpersonal and communication skills.
- Ability to work effectively as part of a team.
- Commitment to improving the lives of families and children.
