2. Job Purpose The aim of the role it to maintain mobility where possible in people who are at risk of deconditioning by delivering the health and wellbeing benefits associated with exercise and group participation within the care home population. The Exercise and Wellbeing Coach will work under the supervision of a professional within the Urgent Care and Support Service and in consultation with professional members of the Multidisciplinary Team, will be expected to lead on the delivery of therapeutic exercise and activity interventions within care home setting with residents by: Using skills and experience to plan and provide flexible, service user centred, 1:1 or group activity-based interventions that contribute to meeting the assessed needs of service users and treatment as outlined within their care plan Working independently to implement therapy according to a plan agreed by a registered practitioner and within the overall framework of the services policies and procedures Identifying and delivering progressions to the care plan and completing risk assessments as appropriate Working alongside the multidisciplinary team to facilitate and evaluate therapeutic interventions for service users and carers Undertaking clinically related administration, and to assist with non-clinical tasks to ensure the smooth running of a service to care home residents delegated by an appropriate senior colleague Take the lead on developing and formulating diversional/recreational activity programmes, for both individuals and groups within care homes, working closely with staff and the voluntary sector to support a sustainable offer within Kingston care homes. Key Result Areas Role and accountabilities Work in partnership with service users in restoring their self-confidence and increasing their level of activity under the direction of the Urgent Care & Support Service team members. The post holder will engage with service users who may demonstrate behaviours that challenge or have difficult family situations of relevance. Measure client goals in line with the support plan and to adjust support plans as agreed with the case co-ordinator. Undertake joint visits to service users in the care home with colleagues from other multi-disciplinary teams as directed.
Identify situations where clients are deemed to be at risk, including situations where people may be neglected or abused in accordance with the Inter-Agency Policy and Procedures for Safeguarding Adults. Promote and encourage the benefits of physical activity for service users in accordance with recommended national and international guidelines Contribute to assessments of service users functional strengths following defined processes, maintaining service users dignity and privacy, observing changes both positive and negative, and reporting appropriately. Monitor service users progress, reporting on their psychological, physical, behavioural, occupational and social response to interventions. Adapt and evaluate interventions ensuring that they support the service user and carer to meet their needs, including regard for cultural and linguistic differences, adjusting own approach accordingly.
Facilitate client choice and empowerment through promoting independence, through the timely provision of information, advice, and support to ensure clients maintain independence for as long as possible. Work together with care staff to develop their understanding and skills in the use of exercise and activity interventions to enhance health, independence and wellbeing. Develop guidance available in multiple formats that care homes can refer to and enables them to provide ongoing exercise and activity opportunities for care home residents on an on-going basis. Ensure the highest level of data quality for all information recorded, ensuring that the information is timely, accurate and complete and according to Your Healthcare policies.
Work with care homes and voluntary sector colleagues to develop exercise opportunities within care homes Be aware of own and others health, safety and security at all times. Undertake and comply with individual, environmental and activity-related risk assessments and to communicate issues around risk to others involved in service users care. Plan and provide appropriate activities to meet the general therapeutic, social and recreational needs of service users. Provide service users instruction, direction and guidance during assessment and intervention and adjust own clinical interventions based on clients needs during the intervention.
Service users may have a wide range of physical disabilities as well as cognitive, perceptual and mental health problems. They may have acute or chronic conditions and will be treated individually or in group settings Evaluate effectiveness of the intervention in discussion with supervising member of the MDT and modify programme accordingly. Take a lead for aspects of therapeutic work and to provide support and guidance to others involved with the service users care such as care home staff, exercise volunteers. Ensure the delivery of safe and effective therapeutic interventions for service users Use communication and negotiation skills to establish a therapeutic relationship with clients and identify barriers and possible solutions to communication difficulties.
Promote the value of engagement in social interaction and recreational activity with clients, care home staff, the multi-disciplinary team and others involved in each persons care Feedback effectively to members of the Urgent Care & Support Service team and external agencies on clients performance and progress towards identified exercise goals. Keep detailed notes, recording data accurately, ensuring all forms and other documentation is completed correctly and up to date in line with Your Healthcare policies and procedures Comply with all relevant national and local policies and procedures. Respect the individuality, values, cultural and religious diversity of service users and contribute to provision of a service sensitive to these needs.
