We have an exciting opportunity to join the Transitions Team as an Experienced Social Worker. In this role, you will be responsible for managing a caseload, supervising a team of four assistant care managers as well as sharing the transitions duty manager role.
Your next role
Within the Transitions team, we work closely with young people between the ages of 14 years and 25 years who may have a learning disability, physical disability or mental health needs and support them to prepare to enter in to adulthood and develop their independence as much as possible. This enables you, as a professional to get to know a young person and their family over a longer period.
You will be responsible for completing the Care Act Assessment in preparation for the young person turning 18 years old. During this time, you will be working in a multi-disciplinary framework with education, health, children services and supported employment services amongst others before the young person transitions to Adult Social Care. Following this, you will follow the care management process to review the support plan annually as well as continual planning for the future with the young person and their family.
You will be responsible for managing a caseload that may include capacity assessments, completing DOLS applications and attending MDT meetings.
You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act “for the effective performance of a customer-facing role”).

A practitioner-specific, Swindon-specific induction programme, aimed at providing you with the information you need regarding the policies and procedures of Adult Social Care at Swindon Borough Council.
The induction programme incorporates a blend of face-to-face activities and e-learning modules aimed at consolidating and reviewing your knowledge of the key areas.
These key areas include:
• Understanding how to use our case management system
• Understanding the Safeguarding and Quality Assurance policies and procedures
• Understanding and using a strength-based approach
Improvements include:
• Multi-million pound investment in staff and support
• Significant restructure creating smaller Social Worker teams
• New intuitive IT system to cut down on desk time and make mobile working the norm
• High standards for management practice, support and supervision for staff
• Listening to our Social Workers by holding regular social work forums
• Implementing Liquid Logic and will be using this system from October 2023.
