Temporary Family Support Practitioner

Hours per week: 37 Monday to Friday, normal working hours
This opening assignment is for 2-3 months

A Family Support Practitioner is required to join the Choices Family Support Service (CFSS) team which aims to support young people age 10-17 years and their families to overcome and manage issues regarding mental health, emotional health and well-being and risk of family breakdown and provide one to one support for young people, family work and parents. This is a highly rewarding role where you will be working closely with children and their families, learning about their strengths and challenges and putting together an assessment of their needs. This role can include some early evening work to suit working parents.
Key responsibilities:
  • Support young people to overcome and better manage difficulties they are facing by assessing needs, agreeing a service plan, carrying out agreed work and reviewing on a regular basis.
  • Work with families experiencing complex challenges and be creative in your approaches in engaging families who may be avoidant and resistant to your efforts.
  • Provide reports to referrers which critically analyse progress and outcomes achieved and which will inform decision making on children’s and their families circumstances going forward.
  • Provide a high quality service to children, young people and their families, based on an assessment of need that is outcomes driven. Deliver complex and specialist interventions with children, young people or families on a 1:1 basis, group or community setting.
  • Provide lead case/care planning management, including undertaking assessing, reviewing and managing risks
  • Attend and participate at relevant multi agency meetings, providing a lead role ensuring reflective and analytical practice review
  • Take responsibility for day to day resource planning to ensure high quality services are delivered.
  • Establish and develop effective relationships and channels of communication with children, young people, families, colleagues, community organisations, partner agencies and other external agencies.
  • Retain accurate and contemporaneous records, using various IT systems to capture information and to produce written reports and present these as necessary.

Qualifications:
  • NVQ Level 3 (or Equivalent) - Health and Social Care working with Children and Young people.
  • Access to own car and full driving licence.

Experience & Knowledge:
  • Experience of working with young people and parents, preferable in the 10–17-year old age range.
  • Experience of working with various issues particularly low-level mental health issues (including anxiety, self-harm, low mood) and low self-esteem.
  • Experience of managing cases, in accordance with agreed procedures, and completing assessments, planning and reviewing work to ensure positive outcomes for young people and families.
  • Experience of working within a multi-agency social care, early years or health environment and applying safeguarding procedures.
  • Minimum of 12 months recent experience in a similar role or context and ideally within a public sector organisation.
  • Knowledge and commitment to safeguarding and promoting the welfare of children and young people.
  • Working knowledge of an outcome based approach to planning, delivering and reviewing services.

Skills & Abilities:
  • Skilled in providing a high level of social care practice using a wide range of therapeutic interventions.
  • Skilled in overcoming the challenges in maintaining and developing effective working relationships with external agencies to ensure the best outcomes for children, young people and their families.
  • Good communication skills to ensure best possible service delivery.
  • Ability to keep the child and or young people as the focus and ensure their feelings and wishes are represented in meetings and record keeping.
  • Ability to work in a team and in partnership with various stakeholders and professional advisors and maintain good client relations.
  • Able to produce concise and accurate reports to agreed standards, formats and timelines, which may be used ultimately for formal reviews.
  • Able to work under pressure and have a problem-solving attitude with an ability to bring a range of interventions together to achieve positive outcomes.
  • Effective verbal and written communication skills; ability to communicate with a wide range of individuals including young people. Excellent organisational skills and ability to prioritise tasks effectively, and manage time effectively.
  • Ability to undertake home /site visits ensuring compliance with lone working policies and procedures.
  • Strong IT skills including Microsoft packages or equivalent (e.g. G Suite).
