Our Reablement Support Workers provide vital assistance to individuals in their homes post-hospital discharge. They play a pivotal role in helping customers achieve their goals, working closely with dedicated occupational therapists to co-develop personalised reablement plans. 

As a First Responder Support Worker, you will support with Hospital discharges, and be first point of contact for new customers, assessing their needs, compiling a support plan, and attending to needs such as personal care, food preparation and daily living activities as required. To meet the needs of the service, shifts include early morning, evenings, weekends, and bank holidays, traveling across the Wiltshire area to carry out customer visits. 

We are seeking individuals with a genuine passion for helping others, understanding of customer care needs and the ability to compile comprehensive support plans.  

Successful applicants will receive a welcome payment of £1300 (subject to tax and NI) for your integral role in reducing flow pressure on hospitals. You will receive £650 upon starting and a further £650 upon successful completion of your probation.
