Job description

Hales Group is seeking a Health and Housing Coordinator to join our client’s team on a temporary basis initially for 3-months.

You will work with key partners across the Health and Housing spectrum to support the delivery of long term, safe and healthy housing environments to prevent recurrences of health issues and associated demands on the system.

Your Key Responsibilities as Health and Housing Coordinator will be;

  • To establish  links into  existing Primary care and hospital processes  to be able to identify and support patients with medical illnesses that are made worse by poor housing.
  • To fully collect and analyse reasons for illness related to poor housing and record and report statistical information to support development of related pathways.
  • To develop relationships and provide appropriate training for with local authorities, GP practices, health visitor services, pharmacists, Crisis Action Team, British Red Cross and other Health and care sector service providers as appropriate.
  • To work to facilitate links between community sector organisations, parish, town councils and the health sector to build awareness of the support available through other housing agencies
  • Responsible for maintaining accurate records and information relating to inspections, caseloads on systems and property files.

To be considered for the role of Health and Housing Coordinator you will have;

  • Educated to a degree level or equivalent experience.
  • Be able to demonstrate highly specific knowledge across work procedures and practices in the NHS and/or adults social services
  • Ability to take complex information and translate into easy to understand information for public consumption
  • Experience of working in an NHS setting, and with vulnerable people
  • Experience of engaging stakeholders and undertaking engagement activities to deliver key messages
  • Ability to analyse and solve new and complex problems.
  • Evidence of good communication skills, both verbal and written and capacity to engage with varied audiences and provide information and support

HOURS: Monday to Friday 37 hours (PT will be considered at min of 25hrs per week)

LOCATION: Hybrid working (must be in commutable distance of Lowestoft or Woodbridge, and happy to travel around Suffolk)

SALARY: £32,000 per annum

TERM: Temporary – initially for 3 months
