Job description

There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels: Manage and prioritise a caseload, in accordance with the Health and Wellbeing needs of their population through taking an approach that is non-judgemental, based on strong communication and negotiation skills, while considering the whole person when addressing existing issues Where required and as appropriate, the Health and Wellbeing Coach will refer people back to other health professionals within the Practice Utilise existing IT and MDT channels to screen patients with an aim to identify those that would benefit most from health coaching Provide personalised support to individuals, their families and carers to support them to be active participants in their own healthcare; empowering them to manage their own Health and Wellbeing and live independently through: i. Coaching and motivating patients through multiple sessions to identify their needs, set goals and support patients to achieve their personalised Health and care plan objectives ii. Providing interventions such as self-management education and peer support iii.
Supporting patients to establish and attain goals that are important to the patient iv. Supporting personal choice and positive risk taking while ensuring that patients understand the accountability of their own actions and decisions, thus encouraging the proactive prevention of further illnesses v. Working in partnership with the social prescribing service to connect patients to community-based activities which support them to take increased control of their Health and Wellbeing vi. Increasing patient motivation to self-manage and adopt healthy behaviours Work in partnership with patients to support them to develop their level of knowledge, skills and confidence enabling them to engage with their Health and Wellbeing and subsequently supporting them in shared decision-making conversations Utilise health coaching skills to develop the knowledge, skills and confidence to manage their Health and Wellbeing whilst increasing their ability to access and utilise community support offers Explore and support patient access to a personal health budget, where appropriate, for their care and support.
g. Assess how far a patients Health and Wellbeing needs can be met by services and other opportunities available in the community h. Provide personalised support to patients, their families and carers to take control of their Health and Wellbeing, live independently, improve their health outcomes and maintain a healthy lifestyle i. Develop trusting relationships by giving people time and focus on what matters to them j.
Where required and as appropriate, refer patients to other health professionals within the PCN k. Develop trusting relationships by giving people time and focus on what matters to me l. Manage and prioritise your own caseload in accordance with the needs, priorities and any urgent support required by individuals in the caseload m. Build relationships with key staff in the GP Practice, attending relevant meetings, becoming part of the wider network team n.
Be proactive in encouraging self-referrals and connecting with all local communities, particularly those communities that statutory agencies may find hard to reach o. Be a friendly source of information about wellbeing and prevention approaches p. Help people identify the wider issues that impact on their Health and Wellbeing such as debt, poor housing, being unemployed, loneliness and caring responsibilities q. Help people maintain or regain independence through living skills, adaptations, enablement approaches and simple safeguards r.
Work with commissioners and local partners to identify unmet needs within the community and gaps in community provision s. Support in the delivery of enhanced services and other service requirements on behalf of the Practice t. Support in the rollout and running of Practice Group Consultations u. Deliver training, mentoring and guidance to other clinicians and staff on Health and Wellbeing matters v.
Support virtual and remote models of consultation and support including e-consultations, remote medication review and teleHealth and telemedicine w. Participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through incidents and near-miss events x. Review the latest guidance ensuring the organisation conforms to NICE, CQC etc. y.
Actively signpost patients to the correct healthcare professional z. Provide targeted support and proactive reviews for vulnerable, complex patients and those at risk of admission and re-admission to secondary care aa. Work with your line manager to undertake continual personal and professional development, taking an active part in reviewing and developing the roles and responsibilities bb. Work with your line manager to access regular clinical supervision to enable you to deal effectively with the difficult issues that people present cc.
Undertake all mandatory training and induction programmes dd. Contribute to and embrace the spectrum of clinical governance ee. Attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed ff.
Contribute to public health campaigns (e.g., flu clinics) through advice or direct care. gg. Maintain a clean, tidy, effective working area at all times.
