SalaryThis is an exciting opportunity for both newly qualified and experienced Health Visitors to join our service. You will be a member of a team delivering the mandated contacts for 0-5 to babies, children and families that have been identified as 'universal HCP'. The team will comprise of nursery nurses and be lead by a clinical team leader.
We have been remodelling our service, developing our skill mix and developing robust pathways to support delivery of the Healthy Child Programme 0-5. The Health Visitor service is part of a wider 0-19 service model which promotes a whole family approach to delivery. The post holder will be part of the universal health visiting team which focuses on delivery of the mandated contacts for babies and children identified on the universal HCP pathway, in addition to delivery of other prevention & promotion aspects of the HCP pathway
To provide clinical leadership support and professional direction to the children, young people and families public health team ensuring that high standards and safe levels of intervention which are evidenced base are delivered within a corporate caseload.
To work autonomously within a multi-agency arena to enable children & Young People (predominantly pre-school age) to reach their full potential (physically emotionally and socially) through education and healthy lifestyles.
.Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 286,700, providing inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. Walsall Manor Hospital houses the full range of district general hospital services under one roof. The £170 million development of our Pleck Road site was completed in 2010 and the continued upgrading of existing areas ensures the Trust now has a state-of-the-art Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit facilities. Work is also well underway on its multi-million pound new Urgent and Emergency Care Centre which will house a new Emergency Department (including Children’s Emergency Department), co-located Paediatric Assessment Unit, Acute Medical Unit and Urgent Care Centre. This is scheduled for completion this year.
We provide high quality, friendly and effective community health services from some 60 sites including Health Centres and GP surgeries. Covering Walsall and beyond, our multidisciplinary services include rapid response in the community and home-based care, so that those with long-term conditions and the frail and elderly, can remain in their own homes to be cared for wherever appropriate.
Key Responsibilities
The job holder is responsible for developing, leading, planning and delivering a range of services for children and their families within a corporate caseload.
Provide a public health based service that contributes to improving the health of children and their families in Walsall through partnership working.
During the absence of the clinical team leaders will promote team working at all times and contribute to the delivery of services within the team
Participate in the continued development of the children, young people and family’s public health service, to include meetings and close liaison with cluster lead and to work in partnership with other health care professionals within the Trust and other agencies.
Care may be delivered within a variety of settings to include client’s home, GP practice clinic, virtually or other settings.
Leadership
Assess, plan, delegate, maintain and review caseloads for each team member, identifying needs and skill mix to ensure delivery of service. Analyse and respond to situations at team and one to one level taking account of changing priorities at team and 1-1 level
Represent the service on a range of multi professional forum as requested and agreed with the cluster lead.
Organise and chair weekly team meetings to cascade information and plan delivery of the team workload in the absence of the clinical lead.
Constructively participate in nursing, clinical and managerial meetings as required.
Monitor and report to the clinical lead regarding client and staff safety, accidents and reportable incidents as necessary in line with risk management local and national policies.
Act as a role model, lead by example through practice with positive attitude and presentation
Contribute to the planning of staff training, both mandatory and for professional development, in conjunction with clinical lead.
Undertake team members’ personal development review and clinical supervision, ensure that individual staff development plans are formulated and objectives agreed in line with corporate objectives.
In consultation with clinical lead, influence and manage change within the service utilising research/evidence based practice and audit mechanisms
Communication
Develop and maintain effective communication networks and work collaboratively with key partners, health workers, statutory and voluntary agencies to actively promote national and local public health priorities.
Demonstrate up to date knowledge on legislation and national directives relating to the service is obtained through regular updates from the steering group, Walsall Healthcare NHS Trust, Internet and other sources.
Ensure effective communication with the child, family and others including those whose first language is not English, and LD liaising and co-ordinating interpreting services as appropriate.
Maintain direct communication and liaison with the clinical lead and attend regular meetings
Provide written and verbal detailed information through comprehensive assessment of the child and family to facilitate referrals to specialist services, e.g., Child and Adolescent Mental Health Service (CAMHS).
Work collaboratively to provide antenatal support to individuals assessed as needing early intervention.
To liaise with teaching staff and school medical staff as appropriate, acting as a link between school and home as necessary.
Clinical Skills
Work in close liaison with the designated and named nurse for child protection and the co-ordinator for looked after children.
Work within Walsall’s Safeguarding Children’s Board guidelines and local child protection policies and procedures.
Initiate and chair or participate in multi-agency meetings to plan care for an individual child in need.
Working in partnership with families and other agencies contribute to the assessment framework for children in need / Early Help processes.
Critically analyse information gathered from the child health records and other relevant sources and provide accurate reports for formal presentation, e.g., case conferences.
Work as a member of a multidisciplinary team providing professional health advice in formulating, implementing and evaluating the child protection plan. Attend case conferences and core groups.
Participate in personal clinical supervision with child protection supervisors.
Within the scope of professional practice and using appropriate frameworks, take lead responsibility to assess, plan, deliver and evaluate the health needs of specific groups of children e.g., looked after children, children with medical needs. This may include undertaking nurse led clinics and assessments.
In conjunction with child and family, liaise with members of multi-disciplinary teams. Refer to other specialist service and engage in joint planning with child, family and other identified services.
Liaise and communicate with other services to assess and produce individual care plans and to co-ordinate their involvement in supporting the child in school or other settings.
To promote the need for immunisation and to advocate the importance of prophylaxis care. To undertake the immunisation of children in line with Trust procedures as required.
Use an assessment framework and adopting a team approach, undertake or delegate as appropriate the 8 weeks, 9 month and 27 month assessments with professional support to nursery staff.
When appropriate, be identified as the ‘Named Health Visitor’ for child protection cases, supporting and supervising team members with child protection issues as required.
To undertake the role of Nurse Prescriber within the scope of the Nurse Prescribers formulary and the Trust Guidelines following the appropriate evidence based information for a range of conditions.
Utilise the Promotional guide or Post Natal Scale to communicate with clients implementing a plan of care in partnership with families in need/crisis.
To develop, monitor and evaluate quality standards with Health Visiting Practice, utilising the Clinical Governance Framework to support the provision of a First Class Service. Develop, monitor and evaluate quality standards within the Health Visiting Service. Incorporate clinical guidelines/service protocols, in practice to ensure that best evidence based care, is delivered to the client group.
Teaching/training requirements
Facilitate, design and implement training programmes for staff, parents/family on individual care needs.
Provide pre and post registration student nurses and others with relevant and suitable experiences within children, young people and families public health service, to include teaching and assessment throughout their placement
Attend regular mentor and assessor workshops to keep up to date with pre and post registration student nurses issues.
Provide mentorship and guidance on clinical and professional responsibilities to team members.

Contribute to the training of parents, families and other professionals on the use and correct understanding of technical equipment.
Take lead responsibility for a specified area of service delivery acting as a resource, trainer and practitioner as appropriate
Health promotion and public health
Identify the public health needs of children and young people (predominantly pre- school age) within the integrated team/defined caseload and determine the priorities for action.
Develop and coordinate health promotion and public health strategies to address identified needs taking into account local and national priorities. For individuals and groups independently and in partnership with others.
Support the implementation of the Child Health Promotion Programme.
Contribute to and influence the planning of Personal, Social and Health Education programmes and participate in the delivery as appropriate.
Provide information that influences the formation of health and social policies within the community.
Facilitate and lead group work with parents, children, young people and others around identified need
Undertake the Child Health Promotion Programme, including primary visits in accordance with Walsall Healthcare Policy
