Health & Wellbeing Coach- Stop Smoking

Conducting one-on-one consultations with participants to assess their smoking habits, readiness to quit, and individual needs. Developing personalized quit plans tailored to each client's goals and preferences. Providing ongoing support and encouragement to clients throughout their quit journey. Delivering evidence-based smoking cessation interventions and techniques, including counselling, behavioural therapy, and pharmacotherapy recommendations.
Monitoring and evaluating client progress, adjusting strategies as needed to ensure successful quit attempts. Collaborating with other healthcare professionals and community organizations to promote smoking cessation initiatives and resources. Maintaining accurate records of client interactions and program outcomes Liaise with professionals including GPs and primary care staff to promote and generate referrals to the service.
