Job description

As early adopters of new insights in technologies, techniques and treatments for improving health we are keen to expand on this and are seeking to employ two dynamic and enthusiastic Heart Failure Specialist Nurses to join our team at Imperial. The successful candidates should be experienced practitioners who are competent in managing Heart Failure (HF) patients in a variety of different settings. They will work as integral members of the HF nursing team but the main focus of these new and exciting roles will be to progress the running of our current HF Virtual Ward (VW), HF remote monitoring service and same day emergency care HF services. These nurses will work closely with our existing NWL Virtual ward hub so will need to be familiar with NHSE guidance on provision of VWs.
These new fixed term 1yr posts will complement our already well established, integrated HF service which provides specialist support and education for our colleagues managing HF patients within both primary and secondary care. We currently provide inpatient HF reviews across our 3 acute hospital sites, one of which has a specialist Heart Assessment Centre. In addition we provide a community HF service, through home visits and community clinics, for our diverse local population. The service is fortunate to be led by a committed Heart Failure Consultant Cardiologist, Heart Failure Nurse Consultant and a Heart Failure Lead Specialist Nurse with specialist input from our Heart Failure pharmacist and psychologist and with access to a cardiac rehabilitation service.
  • Providing ongoing multi-disciplinary input for patients being discharged from hospital but still requiring a higher level of intervention.
  • Act as main point of contact for the Virtual ward hub.
  • Onboarding patients, in a variety of different settings, to our remote monitoring platform.
  • Liaise with rapid response teams and build community pathways to assist with expedited discharge and admission avoidance.
  • Coordinate care for patients in their own homes where required.
  • Establish a close working relationships with same day emergency care (SDEC), emergency and acute medical services for admission avoidance.
  • Data collection/evaluation in relation to SDEC/VW.
  • Follow NICE guidelines for the management of acute and chronic Heart Failure and support the service in achieving National targets
  • As part of the multidisciplinary Heart Failure team to help develop, implement and evaluate a seamless integrated specialist service, ensuring that patients receive the highest standard of evidence based care.
  • Demonstrate a high level of expertise within the service, providing specialist advice, education and support to patients and other health care professionals.
  • Provide specialist nursing advice to support and facilitate high quality, individualised care to patients/clients and relatives/carers without supervision but in collaboration with the multi-disciplinary team and in line with local and national targets
  • Be responsible for contributing to the ongoing development of clinical practice, teaching, research, clinical audits and standards of care within the service, including development of policies, protocols and guidelines.
  • Contribute to service developments in collaboration with multidisciplinary colleagues
