Job summary
This is a new and exciting opportunity for a Heart Failure Specialist Nurse to work on a 12-month 'test of change' in the Worthing locality covering 3 PCN teams; Coastal & South Downs Partnership, Cissbury and Central Worthing.
You will be supporting ECHO confirmed Heart Failure patients who have been newly diagnosed as well as existing patients with an exacerbation of their Heart Failure, in a community setting.
Main duties of the job
Our ambition is to provide a 12-month Heart Failure service Mon- Friday 8.30 -16.30 which improves health outcomes, is patient focused and strongly linked into the localities it serves and delivers a reduction in avoidable use of secondary care.
You will play an active role in the planning, provision, improvement and development in supporting Heart Failure patients in the community, using advanced clinical expertise. You will clinically support and co ordinate in the development of new services across organisational boundaries.

  • Advanced Physical Assesment
  • Extensive Clinical experience of 5 years or more and 2 years in Heart Failure at B6 or above
  • Post Registration Qualification in Heart Failure preferable MSC
  • Independent Nurse Prescriber (V300)
  • Experience of working in an autonomous role

Desirable
  • Experience of project management
  • Experience of clinical leadership in a service

Experience

  • Experience of case management including complex long term conditions and the acutely unwell patient in the community setting
  • Experience of working in an autonomous role
  • Ability to use advanced clinical assessments skills to assess complex patient conditions and formulate, adjust and implement appropriate care plans
