About the Service - Housing & Homelessness Services

A safe home is the solid base from which you build your life. We support people who are experiencing homelessness, or at risk of becoming homeless, as well as providing specialist accommodation and outreach services for rough sleepers, veterans, survivors of domestic abuse, women and young people.

About the Job

We are seeking a professional and welcoming individual to join our team as a Homeless Pathway Co-ordinator on a fixed term basis. The Homeless Response Service and Somewhere Safe To Stay delivers homelessness support in Northumberland and North Tyneside. Working as part of a larger cross borough team, the service responds to sightings and referrals of homelessness, supporting people out of homelessness into temporary and permanent accommodation.

Working as part of a team of Homeless Pathway Co-ordinators covering Northumberland and North Tyneside, supporting rough sleepers, preventing homelessness and moving people through homeless assessments to safe and secure accommodation.

You will support and engage rough sleepers, respond to rough sleeper sightings and participate in early morning and evening outreach with colleagues as and when needed.

The role will involve supporting those at risk of or currently rough sleeping into our own assessment beds. Whilst in these beds, you will continue to support, co-ordinate relevant assessments such as Adult Social Care and take a direct role in the Personalised Housing Plan for that individual. You will intensively support people to navigate out of homelessness and reduce repeat homelessness.

This role offers a varied routine working with a dedicated team covering a large geographical area of the North East. Please note, a valid UK driving licence is essential due to the nature of the role.
