Akton recruitment is currently working closely with local authorities and we are looking for an experienced Hospital Discharge Coordinator in the Sutton area. The role is 3 - 6 months contract.
We are looking for a Social Worker, with ATM level experience, who has managed Hospital Discharge (including NHS CHC) within a busy general hospital environment. We require the candidate to be on site 5 days per week, we are aware a lot of candidates are looking for a hybrid arrangement at the moment but the right candidate will love and thrive in the busy hospital environment. The successful candidate will be required to lead the daily huddles on-site in the transfer of care hub in St Helier (Wrythe Ln, Sutton, Carshalton SM5 1AA)
