Key Responsibilities:
- Conduct Initial Mental Health Assessments and develop Support Plans
- Understand and manage Delayed Transfer of Care (DTOC) cases
- General Case Management and needs assessments
What We're Looking For:
- Qualified Social Worker with 2+ years of experience in adult mental health (required)
- SWE registration (required)
- Professional qualification in Social Work (required)
- Understanding of Hospital Discharge processes and DTOC (required)
