Job description

Hospital Lead Social Work

Lincolnshire County Council is looking for a dynamic and forward thinking Hospital Lead to be part of our aim to provide an outstanding hospital discharge service to our residents and their families.  We work closely with our partners to deliver our high standards of assessment and support in a service that is developing and growing both locally and nationally.  This is a fantastic role that can improve the lives and experiences of Lincolnshire people, enabling the teams to put the individual at the heart of all they do.

About the Role

Lincolnshire Adult Social Care has an exciting opportunity for a Hospital Lead to manage our Hospital Social Work Teams at Pilgrim Hospital Boston and Peterborough City Hospital.  If you have strong leadership and management skills and are looking for a new challenge managing a team of highly focused, professional and hard working Social Care staff, then this is the job for you and we would love to hear from you. 

This post is being created to work alongside the current Hospital Lead who we will be extremely sorry to see retire later this year.  This allows for a planned, supported induction and handover, ensuring minimal disruption to the team and its work, while also ensuring the new Hospital Lead is confident in leading the team forward.

About the Teams

Lincolnshire County Council is committed to providing an excellent service to the people we support and promote an integrated and strengths based and person-centred approach.  We have led the way in developing a fully integrated Discharge to Assess Hub to support swift access to the right services, reducing delays and delivering benefits.

The aim of the teams is to work hand in hand with all system partners to ensure that people we support are at the very centre of their own discharge. We think creatively when planning support packages, utilising “outside of the box” thinking and solutions to ensure an individual care plan is in place that empowers the service user to be an active voice and participant. Our highest continuing aim is to provide the best service possible to service users and their families.

You will:

  • work closely with the three Lead Practitioners in your teams, to support your staff in a fast moving, pressured but very rewarding environment to ensure that the residents of Lincolnshire who are in hospital and have care and support needs are discharged using the Home First Principles.
  • ensure that your teams of qualified and unqualified Social Care staff are enabled to develop their skills and knowledge
  • work collaboratively with health, housing, voluntary and independent sector services and colleagues in the community adult social care teams to ensure delivery of an individual, high-quality service that facilitates discharges in a safe and timely way.
  • promote excellent working practice through on-going quality improvement and promoting the rights of those we work with in line with relevant legislation, guidance and policies and procedures

About You

You will hold a professional qualification and registration in Social Work, Occupational Therapy or Nursing and experience of leadership and management or have a management qualification with relevant Social Care experience.

Please note for this role, you will be expected to be able to travel independently between sites and cover for other leads in the service as required.

You will work as part of the 7 day Adult Care rota, this equates to approximately 1 in 6 weekends and these are all scheduled at least three months in advance.
