LB Merton, Adults Social Care staff are given opportunities attend conferences, workshops and have access to research and practice resources to support their ongoing professional development
We currently have a number of exciting opportunities for confident and skilled Social Workers to support us in delivering our service for adults in need of social care. Our approach is strength-based, and we aim to achieve the best outcomes - enabling people to be safe, as independent as possible and to live the lives they want.
Reporting to a Team Manager or an Assistant Team Manager,Social Workers are responsible for assessment, support planning and safeguarding vulnerable adults. You will work with service users, family carers and commissioners to ensure that support packages comprehensively meet assessed needs, whilst also being innovative and cost-effective.
Hospital Team
Our social care staff within the hospital team work closely with NHS colleagues from acute and rehabilitation facilities, and with community health services, to facilitate safe, timely discharge. Their priority is to enable people to return to their own homes, utilising reablement and other services to enable them regain independence.
