Job description

Adferiad Recovery delivers a flexible and coordinated response to the exceptional circumstances faced by people with co-occurring mental health and substance use conditions and related issues. Vulnerable people facing complex life challenges need consistent and seamless support to ensure they remain engaged with vital health and wellbeing services – and to prevent them becoming disenfranchised and isolated.

Adferiad Recovery harnesses the talents and experience of long-established and leading Welsh charities CAIS, Hafal and WCADA. Our combined expertise in the fields of substance use, mental health, housing, criminal justice, and employment support enables Adferiad Recovery to meet the needs of our most vulnerable with a single, unified, and comprehensive approach.

Contract: Permanent, 30 hours (Flexible shift pattern 9am till 8pm over 7 days)

Salary: £21,507.75 per annum

Job Location: Borough of Bridgend

Adferiad are working with BCBC and Partners to deliver Housing First (HF). Housing First is a unique approach aimed at helping to tackle homelessness by supporting people through community-based support for individuals with complex needs across the County Borough of Bridgend.

Bridgend Housing First projects support focuses on each person’s strengths and aspirations enabling them to feel safe and secure with the long-term aim to reduce harm, achieve a tenancy and sustain their home and independence in their community.

This is a brilliant opportunity to work with service users in areas such as employment, training and education, debt management and daily living skills to promote wellbeing and a healthy lifestyle.

Support will be available between 9.00 am to 8.00 pm - Mon to Sun with specific hours of support determined by each client’s needs.

Staff will be expected to be flexible and work across the hours and days of service delivery and will be part of a rota of on-call service for out of hours provision where necessary.

Responsibilities:

Provide a variety of person centred, tailored interventions that support individuals with complex needs to find sustainable accommodation and develop their independent living skills.

  • Working with service users in a proactive time and task specific manner on a variety of issues to impact positively on issues relating to housing, employment, training and education, offending behaviour and social isolation.

  • Practice homeless prevention, tenancy sustainment, Psychologically Informed Approaches, Solution Focus Practice, harm reduction, and co-production.
  • Manage a client caseload, and complete individual needs assessments, risk assessments, support plans and offer practical advice and advocacy to service users.
  • Build and maintain positive relationships with individuals with complex needs to ensure successful support, to address the persons accommodation and wider support needs.
  • Support service users to exercise informed choices about their lives where possible and to enhance their social skills and participate in their community.
  • Support service users to identify the areas of support needed and identify the best option of support based on ongoing needs as outlined in their individual Support Plans.
  • Work with service users to achieve their outcomes and the outcomes of the project, employing creative and innovative methods to meet these outcomes and, through their experience and resources, bring added value.
  • To provide a service to the service user group that complements and enhances the service provision of the local housing providers and specialist substance misuse agencies in the context of the Housing Support Grant initiative.
  • Liaise with relevant services, organisations and agencies with regards to accessing additional support for service users.
  • To maintain up to date case management and review records as required, alongside the case management policy.
  • To participate in the provision of an on-call service.

Person Specification:

  • There are no specific qualifications but there must be evidence of commitment to personal and career development relevant to the post.
  • Qualified / willing to work towards NVQ Level 2 in Health and Social Care or hold equivalent qualifications.
  • Experience of providing care and support services to individuals with complex needs in a community setting (such as those who are homeless/at risk of homelessness, offending behaviour, mental health, domestic abuse, substance misuse).
  • Knowledge of Housing, offending and employment issues.
  • Full driving licence and full access to a vehicle for work purposes.
  • Welsh Language Skills (desirable)
