One of the services they have is working alongside local hospitals, offering a discharge service to those who are homeless.
Your new role
As a Support Worker you will be working in an accommodation-based service providing housing related support to adults who are experiencing homelessness. You will act as a keyworker, encouraging positive and healthy lifestyles and assist individuals to develop skills for independent living. You will work closely with partners across the city and take a multi-agency approach to reach positive outcomes for individuals.
What you'll need to succeed
You must have a can do attitude, be willing to go the extra mile and be able to think outside the box. You will work on the internal IT system ensuring that all paperwork is accurately recorded and up to date. You must be able to make decisions with the sole purpose of supporting the people we work alongside to achieve their goals to create positive outcomes.
