Housing Support worker (Hospital Discharge)

We are seeking a Settle-In Worker to work intensively with service users in the period following Hospital Discharge. This innovative role is part of a pilot project to re-model the current Hospital Discharge pathway.
The Settle-In Worker will play a pivotal role providing practical support to vulnerable people who are rough sleeping or at risk of rough sleeping when they are discharged from hospital.
*DBS On the update service required and experience in the mental health support sector
Location: Bristol (HYBRID)
Contract duration: 3 Months+ Temp to Perm
Hours: 37.5
Salary: £15.03 -17.92 DOE (VIA UMBRELLA)

Day to day responsibilities include:
- Building trusting relationships with service users to support them in sustaining accommodation, improving health & well-being, and preventing a return to the streets or avoidable hospital attendance.
- Maximizing service users' income and assisting with applications for sustainable housing.
- Supporting service users with household tasks, digital inclusion, accessing social networks and community support.
- Assisting with access and engagement with substance use and community mental health services.
- Working closely with carers and other services involved with service users.
- Providing proactive and responsive support, including preparing accommodation and arranging essentials prior to discharge.
- General duties include attending meetings, supervision, comply with policies/legislation,
Essential criteria include:
- DBS on the update service or made with in the last year
- Have a car and a valid UK driving licence
