Job description

To co-ordinate and provide a full administration service to the nursing team.
Managing patients and professionals through defined pathways to enable patients are seen by the right person, at the right time and in the right place, ensuring that the quality standards of the Trust are met.
To provide support and guidance to patients, carers, relatives and others making contact with the services.
To deliver a Patient Pathway in accordance with Local and National Guidelines in relation to Referral to Treatment (RTT 18 weeks). To prospectively manage the clinic and clinic slot availability to ensure maximum efficiencies within the service to agreed templates, minimising any impact on clinic utilisation in a timely manner.
We are looking for a highly motivated person to join our current ILD service coordinator, within the interstitial lung disease team based at the Glenfield hospital. We are a multi-disciplinary team of admin, nursing, pharmacists and consultants members. We run clinics throughout the week and have a multi-disciplinary team meeting every Wednesday to discuss our patients.
• To receive referrals and make new and follow up appointments
• To ensure full utilisation of clinic slots available.
• To validate appointment details on the HISS system as defined by scheduling rules.
• Liaise with clinicians regarding availability for clinics.
• To monitor, maintain and report clinic cancellations, including notice time given and utilisation time available..
• Process patient DNA’s and cancellations in accordance with relevant pathway guidelines liaising with clinicians to ensure discharge letters to the GP are completed.
• Manage the weekly MDT, coordinating lists, taking minutes and recording outcomes of patients requiring MDT input. Obtain notes and ensure decisions are documented, filed and inputted on the appropriate clinical and national databases.
• Maintain clinic scheduling on HISS, including cancellation and re-scheduling of clinics, setting up of new clinic templates and ad-hoc clinics
• Any other duties commensurate with the role and service needs.
