PRIMARY DUTIES AND RESPONSIBILITIES Within their individual PCN Practice, the Junior Pharmacist will: Manage monthly high-risk drug monitoring searches. Lead in the delivery of the PQS audit activity. Manage day-to-day medication related queries coming through the Practice via telephone and online, from patients, community pharmacies and hospital pharmacies. Reconcile medications in patients discharged from hospital who have had multiple medication changes, and organise appropriate follow up if needed.
Manage prescribing alerts, including running searches, disseminating relevant information to the team, and logging any actions undertaken on a PCN digital platform. Regularly review CQC medications and prescribing related searches. Triage medication review forms completed by patients via the practices online consultation platform. Promote and review the use of the Community Pharmacy Consultation Service (CPCS).
Undertake medication reviews over the telephone, via video, on-line consultation platforms and in person. Attend regular supervision sessions with the senior pharmacist. KEY TASKS Repeat prescribing process repeat prescriptions in accordance with the PCN Practices repeat prescribing policy.
Ensure patients have appropriate monitoring tests in place when required. Undertake medication reviews over the telephone, via video and on-line consultation platforms and in-person. Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure patients receive the medicines they need post-discharge. Contribute to reductions in medicine related hospital admissions and re-admissions by supporting patients to get the best outcomes from their medicines and identifying and addressing medicines related issues.
Manage patients, Practice staff and other health care professionals medicine queries. Participate in clinics where medicines are the main interventions e.g. smoking cessation, anti-coagulant monitoring clinics, high-risk drug monitoring. Contribute to patient facing multi-morbidity long-term condition reviews and multi-disciplinary reviews.
Review daily Pathology results for patients on known medicines. Act as a source of medicines information for all of the Practices teams and patients (e.g. around doses, side effects, adverse events, possible alternatives if stock issues). Undertake minor ailments triage: deal with minor ailments and triaging patients appropriately working within scope of Practice and limits of competence.
Signposting to community pharmacy and referring to GPs or other healthcare professionals, where appropriate. Process patient-facing domiciliary and care home medication reviews and produce recommendations for GPs or nurses on prescribing and monitoring. Work with care- home staff to improve safety of medicines ordering and administration Ensure patient safety when they are transferred between care providers through reconciliation of prescribed medicines. Care Quality Commission: work with GPs to ensure the Practice is compliant with CQC standards with regard to medicines.
Support public health campaigns: provide specialist knowledge on all public health programmes available to the general public. Foster good collaborative working relationships not only within PCN Practices but also with external colleagues e.g. community and hospital pharmacists, PCN, ICB. May be required to work extended hours (late evenings / weekends).
