Band 8a - Lead Pharmacist Intestinal Failure
An exciting opportunity has arisen within our pharmacy department at the Royal Berkshire NHS Foundation Trust for a Lead Intestinal Failure Pharmacist. We are recruiting an enthusiastic, highly motivated, dynamic pharmacist. The individual will be responsible for delivering, managing, and developing a proactive, high-quality, cost-effective, safe pharmacy clinical to patients requiring Parenteral Nutrition in the inpatient, outpatient, and homecare settings.
The Royal Berkshire NHS Foundation Trust Pharmacy Service aims to proactively support staff, service users and carers in achieving safe and effective medicines management, optimising the use of medicines by providing a high quality and accessible service.
We do this by providing:
Accurate and independent information and education about medicines to other healthcare professionals, service users and carers.
Clinical activities to facilitate the management of medicines by service users within inpatient and community teams.
Support to ensure that medicines management resources are used cost effectively within Royal Berkshire NHS Foundation Trust and across the local health community.
To develop and implement Pharmacy policies and procedures as appropriate to this role and area of expertise, including clinical policies, clinical guidelines and PGDs.
Be available for expert consultation with medical and nursing staff on matters relating to safe, economic, and effective drug usage, including implementation of Infection Control policies, Medicines Management Policy and the Trust Formulary.
Provide advice on legal, ethical and professional matters relating to medicines.
Participate in clinical pharmacy services including training, prescription chart review, patient counselling, adverse drug reaction reporting and therapeutic drug monitoring and individualisation of patients’ drug therapy.
Undertake clinical and technical work within the dispensary or ward as a clinical pharmacist according to service requirements.
To ensure implementation of pharmaceutical aspects of relevant National Guidance, in liaison with the Clinical Services Manager.
Participate or manage the input into MDT ward rounds and cross-sector MDT working, where appropriate.
Support outpatient clinics with pharmaceutical advice, as both a hub and spoke site, to ensure safe and effective clinical treatment for all Trust patients wherever they are seen.
To work as an independent prescriber in the clinical field pertaining to this job description.
