Lead Practitioner

Lincolnshire County Council has an exciting opportunity in for Lead Practitioners within our Hospital Service. This is an excellent opportunity to work within the hospital discharge and transfer of care service.

Following the successful recruitment of a Lead Practitioner for the Adult Care Team at Pilgrim Hospital, Boston, we are now able to recruit a further Lead Practitioner to give additional management support to this growing Adult Care team.

This role will require you to work closely with the Lead Practitioner already in post to enable the development of social care staff, both qualified and unqualified. Collaborating equally to ensure continued learning, innovation, support, and delivery of a high-quality service. You will be supporting and encouraging the team to have the confidence to work in a strengths-based way.

The aim of the team is to work hand in hand with NHS and third sector staff to ensure that people we support are at the very centre of their own discharge. We think creatively when planning support packages, utilising “outside of the box” thinking and solutions to ensure an individual care plan is in place that empowers the service user to be an active voice and participant. Our highest continuing aim is to provide the best service possible to service users and their families.

LCC’s Hospital Service is leading the way on the introduction of a fully integrated Discharge to Assess Hub, to support access to the right services in a more fluid way, reducing delays and delivering benefits to service users.

You will work as part of the 7-day Adult Care rota. This currently equates to approximately 1 in 5 weekends worked, and these are all scheduled 3 months in advance.

About You

It goes without saying that you must be a professional Social Worker, with significant experience to undertake this role.
