Job description

To work as a member of the Specialist Palliative Care Multidisciplinary Team, planning and facilitating a range of creative, social and diversional activities that enhances the emotional, social, spiritual, cognitive and physical well-being of individuals, with a particular focus on Life Celebration work. To develop and support a team of volunteers to support in the delivery of the above To develop and sustain collaborative working with external arts and leisure facilities and funding bodies. Main Responsibilities: Clinical Supporting patients in LWS, IPU or Community to create Life Celebration work through a range of different media, e.g. letters for family members, memory boxes, life story work.
Support and development of a team of volunteers to deliver creative and diversional activities to individuals and groups and support in Life Celebration work. Provide training, support and supervision for volunteers assisting patients with creative and diversional activities and Life Celebration work. Actively contribute to multidisciplinary discussions and patient care planning. Supported by volunteers, assist and support patients with life limiting illnesses and their carers where appropriate to undertake a range of creative and diversional therapies.
Works with the MDT, undertaking comprehensive assessments of individuals diversional, creative and leisure related needs and abilities. Supported by volunteers, plan and implement individualised patient-centred activity programmes to individuals, families and other groups in the inpatient unit, LWS and at home. Supported by volunteers, facilitate patient choice and decision making through offering a range of options for creative and diversional activities. Evaluate individual and group programmes of creative and diversional activities.
Supported by volunteers, promote well-being and living positively through distraction or purposeful activity. Provide mentorship and practice supervision of visitors and student placements in the service. Ensure all IT and other equipment is updated and maintained to provide required level of resource for project work, for example video diaries, web based projects. Work flexibly across service areas to meet service needs.
Ensure that all duties are carried out to the highest standard, and in accordance with current quality initiatives within the work area. Operational With the support of the volunteers, maintain adequate supplies of art and craft materials, ensuring that resources are used efficiently. Undertake delegated responsibility to manage resources within an agreed budget. To be aware of the responsibilities of all employees to maintain a safe and healthy environment for patients, visitors and staff. Communication Develop and sustain collaborative working with external arts and leisure facilities and funding bodies. To effectively communicate with patients, relatives, carers especially where there are barriers to understanding, requiring empathy, reassurance and persuasive skills and the multidisciplinary team. To participate in MDT meetings.
Under the guidance of the LWS Team leader, liaise with multi professional agencies, outside the Charity to maintain effective working relationships. Liaise very closely with the LWS Band 6, and where required the LWS Team leader, communicating any problems.
