Job Description:
We are seeking motivated and adaptable Locum Band 5 Community Nurses to join our healthcare team for Ad Hoc Shifts. As a Community Nurse, you will provide essential nursing care and support to patients in their homes and within the community. Your role will involve a variety of clinical tasks and responsibilities to meet the diverse healthcare needs of our patients.
Key Responsibilities:
Patient Assessment and Care:
- Conduct comprehensive assessments of patients in their homes to determine their healthcare needs.
- Administer prescribed medications and treatments, ensuring accurate documentation.
- Provide wound care, catheter care, and other clinical procedures as required.
- Monitor vital signs, observe for changes in condition, and promptly report any concerns to the appropriate healthcare professionals.
Health Promotion and Education:
- Educate patients and their families on managing their health conditions and promoting wellness.
- Offer guidance on medication management, dietary needs, and lifestyle adjustments.
- Provide support for patients with chronic illnesses to help improve their quality of life.
Medication Management:
- Prepare and administer medications, adhering to established protocols and safety guidelines.
- Ensure medication compliance and provide education on drug interactions and potential side effects.
Documentation and Reporting:
- Maintain accurate and up-to-date electronic health records (EHR) for each patient encounter.
- Complete incident reports and communicate patient status changes to the nursing team and healthcare providers.
Collaboration and Communication:
- Collaborate with multidisciplinary healthcare teams, including physicians, therapists, and social workers, to coordinate patient care.
- Communicate effectively with patients, families, and caregivers to address their concerns and questions.
Safe and Ethical Practice:
- Adhere to professional standards, ethical guidelines, and infection control measures.
- Ensure the safety and well-being of patients during all care interactions.
Ad Hoc Shifts:
- Be available for Ad Hoc Shifts as needed, including weekends.
- Maintain flexibility in scheduling to accommodate patient needs and team requirements.
Qualifications and Requirements:
- Registered Nurse (RN) qualification with valid nursing license.
- Community Nursing experience.
- Current Basic Life Support (BLS) certification.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Proficiency in electronic health record (EHR) systems.
- Compassionate and patient-centered approach to care.
- Ability to work independently and as part of a healthcare team.
- Reliable transportation and willingness to travel to patient homes within the community.
