Job Description:

We are seeking motivated and adaptable Locum Band 5 Community Nurses to join our healthcare team for Ad Hoc Shifts. As a Community Nurse, you will provide essential nursing care and support to patients in their homes and within the community. Your role will involve a variety of clinical tasks and responsibilities to meet the diverse healthcare needs of our patients.

Key Responsibilities:

Patient Assessment and Care:

• Conduct comprehensive assessments of patients in their homes to determine their healthcare needs.
• Administer prescribed medications and treatments, ensuring accurate documentation.
• Provide wound care, catheter care, and other clinical procedures as required.
• Monitor vital signs, observe for changes in condition, and promptly report any concerns to the appropriate healthcare professionals.

Health Promotion and Education:

• Educate patients and their families on managing their health conditions and promoting wellness.
• Offer guidance on medication management, dietary needs, and lifestyle adjustments.
• Provide support for patients with chronic illnesses to help improve their quality of life.

Medication Management:

• Prepare and administer medications, adhering to established protocols and safety guidelines.
• Ensure medication compliance and provide education on drug interactions and potential side effects.

Documentation and Reporting:

• Maintain accurate and up-to-date electronic health records (EHR) for each patient encounter.
• Complete incident reports and communicate patient status changes to the nursing team and healthcare providers.

Collaboration and Communication:

• Collaborate with multidisciplinary healthcare teams, including physicians, therapists, and social workers, to coordinate patient care.
• Communicate effectively with patients, families, and caregivers to address their concerns and questions.

Safe and Ethical Practice:

• Adhere to professional standards, ethical guidelines, and infection control measures.
• Ensure the safety and well-being of patients during all care interactions.

Ad Hoc Shifts:

• Be available for Ad Hoc Shifts as needed, including weekends.
• Maintain flexibility in scheduling to accommodate patient needs and team requirements.

Qualifications and Requirements:

• Registered Nurse (RN) qualification with valid nursing license.
• Community Nursing experience.
• Current Basic Life Support (BLS) certification.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Proficiency in electronic health record (EHR) systems.
• Compassionate and patient-centered approach to care.
• Ability to work independently and as part of a healthcare team.
• Reliable transportation and willingness to travel to patient homes within the community.
