Locum Community Occupational Therapist (Hybrid) - Ongoing Support Team - Trowbridge, Wiltshire - £30-£35 p/h
We are currently seeking a skilled and dedicated Locum Community Occupational Therapist to join our Ongoing Support Team based in Trowbridge, West Wiltshire. The successful candidate will play a vital role in providing ongoing support and rehabilitation services to individuals in the community, promoting independence and improving their quality of life.
Hybrid working is available, however assessments will need to be conducted out in the community, the hybrid ratio can be discussed and negotiated at interview
Negotiable hourly rate around £30-£35 per hour
Assessment and Rehabilitation Planning
- Conduct comprehensive assessments of individuals' physical, cognitive, and psychosocial needs.
- Develop individualized rehabilitation plans, setting realistic goals in collaboration with clients, their families, and other healthcare professionals.
Occupational Therapy Interventions
- Provide evidence-based occupational therapy interventions to address activities of daily living (ADLs), mobility, and environmental adaptations.
- Implement strategies to enhance independence and quality of life for individuals with a range of physical and mental health conditions.
Assistive Technology and Equipment
- Evaluate the need for and prescribe assistive technology and adaptive equipment to optimize individuals' independence.
- Provide training to individuals and their caregivers on the proper use of recommended devices.
Crisis Intervention and Support
- Respond to crisis situations, providing immediate support and interventions.
- Collaborate with other healthcare professionals to ensure a holistic approach to crisis management.
Home Visits and Community Integration
- Conduct home visits to assess and address environmental factors affecting individuals' independence.
- Work towards community integration by identifying and overcoming barriers to participation.
Collaboration and Communication
- Work collaboratively with the Ongoing Support Team, including physiotherapists, social workers, nurses, and other healthcare professionals.
- Maintain clear and effective communication with clients, families, and external agencies involved in the care of individuals.
Documentation and Record Keeping
- Maintain accurate and up-to-date electronic health records, documenting assessments, care plans, and interventions according to regulatory standards.
- Ensure confidentiality and privacy of individuals' health information.
Professional Development
- Stay informed about current best practices in occupational therapy and participate in professional development activities.
- Contribute to continuous improvement initiatives within the Ongoing Support Team.
Qualifications and Requirements
- Bachelor's or Master's degree in Occupational Therapy.
- Current registration with the relevant occupational therapy regulatory body.
- Experience in community-based occupational therapy, preferably in ongoing support and rehabilitation.
- Strong clinical reasoning and problem-solving skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Knowledge of relevant legislation and policies related to community healthcare.
- Commitment to person-centered care and promoting the autonomy of individuals.
