SalaryAn exciting opportunity has arisen for an experienced, enthusiastic and committed Nurse to join the Paediatric team cross site at Wexham Park and Frimley Park hospital as Matron for Transition.
They will Lead the development and implementation of FHFT Healthcare Children and Young People (CYP) Transition Strategy and workplan aligned to Trust Wide objectives, whilst recognising the bespoke needs of Children and Young People and their Parent/Carers.

Ensure that high standards of transition preparation and transfer of care are implemented, monitored, and reviewed and where necessary develop plans in collaboration with the multidisciplinary team ensuring action is taken to continuously improve patient engagement
and outcome.
Demonstrate effective leadership, setting, monitoring, and maintaining professional transition standards, working collaboratively to manage complaints and lead the nursing contribution for clinical governance at a speciality/department level in line with the Nursing & Midwifery Strategy
