Job overview
An excellent opportunity has arisen for an experienced Mental Health Practitioner to join our Mental Health Liaison Practitioner Service in the Mid Sussex Assessment and Treatment Service. You will work as part of a multi-disciplinary team and provide specialist assessment and short term interventions to patients to promote their recovery and you will have the opportunity to work closely with local practices, IAPT services, Recovery College and other providers in the local area.
Main duties of the job
You must be enthusiastic, self-motivated, flexible and able to work autonomously and we will support you by providing supervision and training including a comprehensive induction, access to complex case formulation and courses to support your continuous professional development.
Working in our team includes:
  • Working autonomously as a Lead Practitioner to a defined caseload of clients. This will include offering specific interventions to people diagnosed with depression and anxiety and other conditions.
  • You will provide assessment and treatment to our service users providing psycho-educational interventions that are time limited.
  • Liaison with primary care workers and GP's, providing outcome of Assessment plans as well as detailed risk assessments
  • You will be an active member of our MDT

This post offers the Trust's £4,000 Incentive for band 5-7 Nurses & Practitioners, as well as a relocation package up to a maximum of £8,000 (subject to terms and conditions).

Detailed job description and main responsibilities
Working as an MHLP includes:
  • Working autonomously to a defined caseload of clients diagnosed with mild to moderate Mental Health problems. This will include offering specific interventions such as psycho-education, CBT informed sessions and short term recovery focused interventions
  • You will complete initial assessments and work closely with our referrals coordinators, GP's and other agencies.
  • Your interventions will be a pre-defined offer of 6-8 sessions.
  • You are welcome to co-facilitate groups if you have an interest in this area.

To work closely with other practitioners, local day services, local education establishments employment agencies and third sector services
To provide information to patients, and their carers, about their care and options available to them.
To ensure that the service is equitable and accessible to clients and carers.
Quality and audit
To ensure that assessment and interventions are provided to agreed standards.
To keep GPs and primary care teams well informed on outcomes of interventions and therapies provided to their patients.

Qualifications/Education
Essential criteria
  • •Registered Mental Health qualification or equivalent
  • •Up to date Registration
  • •Post Basic Courses / relevant Study Days
  • •Evidence of CPD

Experience
Essential criteria
  • •Three years' post-qualifying experience of clinical work with people with a range of Mental Health problems
  • •Experience of partnership working within and across various different statutory and non- statutory teams and agencies
  • •Experience of successfully managing a caseload and prioritising clinical work.
  • •Experience of providing a range of clinical interventions to people with a variety of Mental Health problems.
  • • Experience of working consultatively with professionals
  • •Experienced in conducting clinical assessments, including risk assessments
