Job description

To be a Mental Health Nurseor Mental Health Social Worker within the North West NHS Op Courage Veterans & Reservists Mental Health & Wellbeing Service (Op Courage) delivering the Op Courage service to veterans and “soon to be veterans” in the North West. The post is part office based (in Bury) where responsibilities include: initial triage of referrals as part of the single point of access and participation in a Duty rota including liaising with referrers and other services and providing support to members of the Multi-Disciplinary Team in relation to risk management and safeguarding issues. The other part of the post is the delivery of clinical appointments within a range of venues (including MoD venues) across the North West or a home setting where clients’ needs necessitate this. Clinical appointments include; initial face to face Mental Health, social care and risk assessments (including collecting standardised assessment measures); and case management of an assigned group of clients who require on-going monitoring of their Mental Health and risk management; emotional and practical support; and the delivery of evidence based interventions for depression, anxiety, and anger.
This role ranges from recognising the early signs of Mental Health problems, providing access to early support, dealing with crisis/duty calls to assessment, case management and brief evidence-based interventions. Help is also, at times provided to the veteran for other needs that may affect Mental Health and wellbeing – for example, with housing, finances, employment, relational and social support and reducing alcohol consumption.
The post includes the delivery of clinical appointments within a range of community venues across the Northwest. Clinical appointments include digital or face to face Mental Health, social care and risk assessments and case management of an assigned group of clients who require on-going monitoring of their Mental Health and risk management; emotional and practical support; and the delivery of evidence-based interventions for depression, anxiety, and anger. We also utilise digital platforms for appointments were requested to support flexibility of service delivery.
The post holder will also contribute to a duty rota (in the office base at Bury) where responsibilities include: initial triage of referrals as part of the Op Courage single point of access, liaising with referrers and other services and providing advice and support both to other members of the MVS and to other professionals working with veterans with Mental Health difficulties in relation to clinical management and safeguarding issues.
Here at Pennine Care Trust NHS Foundation Trust we are looking to expand the North West NHS Op Courage Veterans & Reservists Mental Health & Wellbeing Service (Op Courage) and seeking to employ an enthusiastic and motivated Mental Health Nurse who is interested in military veterans Mental Health.
Op Courage NW is a North West-wide community service for ex serving personnel and those transitioning out of the armed forces with a discharge date. The service offers a single point of entry, assessment and brief intervention/case management service to veterans who are experiencing Mental Health difficulties.
Op Courage NW is co-located with the Military Veterans’ Service - a multi-disciplinary team of practitioners led by a Consultant Clinical Psychologist which is commissioned to offer treatment and interventions to Veterans of the British Armed Forces (including current reservists when not mobilised) who are experiencing complex Mental Health difficulties related to their military service.
Main Duties & Responsibilities
To undertake screening and telephone assessment duties including liaison with referrers and other service providers to ensure clients are treated in the most appropriate service for their needs.
To provide a thorough psycho-social assessment of the client (aged 16+), this will include a risk assessment and a care plan.
To deliver individualised interventions to the client based on individual care plan
Liaison with all members of Op Courage and Military Veterans’ Service, MoD clinicians, Secondary Care Mental Health Services, Voluntary /Statutory agencies across the North West
To manage own caseload by delivering brief interventions to the client, whilst maintaining and allocating new clients.
To deliver agreed clinical contacts as set by service
Clinical
To conduct a through psycho-social assessment of clients referred to Op Courage, to identify their needs and develop a collaborative care plan
To provide case management and brief interventions for clients with moderate and complex Mental Health problems where required focus is generally around risk to self or others, safeguarding children or safeguarding vulnerable adults
Have a good working knowledge of anti-depressants and other medication that is prescribed within Primary Care.
To work jointly with colleagues within Op Courage and Military Veterans’ Service, MoD and other NHS colleagues as required
To assess clients presenting with multiple and complex issues and make decisions regarding the appropriate therapeutic intervention required to meet client’s needs. This may include referral on to another service or modality of treatment.
To assess risk of harm to self/others or property and take action as appropriate. This may involve breaching of client confidentiality in the interest of the clients or others safety e.g. Safeguarding Children/Adults, acts of terrorism etc. This will be assessed in line with Pennine Care NHS Foundation Trust policies
To organise and manage a clinical caseload
To maintain appropriate electronic records using PC-MIS web based clinical system in line with agreed protocols as directed by Line Manager
To maintain effective communication and clear and concise documentation for individualised care as per service protocols and Trust policies
To inform referrer of outcome of assessment and provide a summary of interventions delivered at discharge
To ensure accurate completion of weekly diary of activities for Administration Team
To co-facilitate therapeutic groups where appropriate and requested by Line Manager
To act in accordance with requirements for practice- NMC Code of Conduct or H.C.P.C. Code of conduct
To actively participate in clinical and management supervision
To provide a resource for discussion for other members of the MDT
Facilitate communication with colleagues by active participation in team and clinical meetings as required by Team Manager.
There is an expectation for you to deliver services across the North West. This may include home visits.
Research and Service Development
To participate in ongoing audit/evaluation of the Op Courage and Military Veterans’ Services and submit activity and clinical outcome data as directed by Line Manager
To routinely input onto Service databases as directed by Line Manager
To utilise research findings in the promotion of good practice, clinical effectiveness and audit.
To follow Pennine Care NHS Foundation Trust, Op Courage and Military Veterans’ Service policies and protocols
To participate in service development where agreed with Line Manager
Teaching, mentoring and supervision
To work within Op Courage and with other Mental Health services in the North West:
As a resource for discussing veteran clients’ needs
As a resource for education and training
To offer shadowing opportunities for students on placement as directed by Line Manager
To undertake Continuing Professional Development and participate in military Veterans’ Service CPD sessions
To provide supervision of other professionals as agreed with Line Manager
To maintain a high quality practice by attending clinical supervision in line with Pennine Care NHS Foundation Trust policies and as agreed by Military Veterans’ Service policies and protocols
General Duties of All Postholders
To undertake any other reasonable duty, which is appropriate to the band, when requested by Senior Staff
To be familiar with and comply with all Trust and departmental policies, procedures, protocols and guidelines

Standards of Business Conduct

Safeguarding
Appointments to regulated and controlled activities require an enhanced CRB disclosure and registration with the Independent Safeguarding Authority (ISA), Vetting and Barring Scheme. Checking will be in accordance with the timeline promulgated by the Independent Safeguarding Authority.
All staff have a responsibility to promote the welfare of any child, young person or vulnerable adult they come into come into contact with and in cases where there are safeguarding concerns, to act upon them and protect the individual from harm.
All staff should refer any safeguarding issues to their manager and escalate accordingly in line with the Trust Child and Adult Safeguarding Policies.
All staff should familiarise themselves with the NICE Guidelines “when to suspect child maltreatment 2009.”
Professional and Personal Development
The post holder must ensure that they are aware of their responsibilities by attending the Trust Mandatory Training and Induction Programme
The post holder will be involved in a formal IPDR/KSF review with his or her manager at least every 12 months. Once performance / training objectives have been set, the staff member’s progress will be reviewed on a regular basis, so that new objectives can be agreed and set, in order to maintain progress in the service delivery.
Confidentiality & Information Governance
Confidentiality is of prime importance. In the normal course of duties, the post holder will have access to confidential documents and information relating to patients, service users, staff and contractors, as well as information of a commercially sensitive nature. Such information should not be communicated to anyone outside or inside the NHS unless done in the normal course of carrying out the duties of the post. Disciplinary action will be considered where a breach of confidence has been established
All information obtained or held during the post-holders period of employment that relates to the business of the Trust and its service users and employees will remain the property of the Trust. Information may be subject to disclosure under legislation at the Trust’s discretion and in line with national rules on exemption
The post holder must maintain high standards of quality in corporate and clinical record keeping ensuring information is always recorded accurately, appropriately and kept up to date . The post holder must only access information, whether paper, electronic or in other media, which is authorised to them as part of their duties
The post holder must ensure compliance with the Data Protection Act 1998
