Location: Longford Primary Care Centre, Longford Road, Coventry CV6 6DR

Ward: Memory Assessment & Community Dementia Service

Pay Rates: £21.80 per hour

Shift Patterns: 37.5 Mon - Fri 8.00 4.00pm (30 min break)

What you can add:

• Make a significant and specialist contribution in the memory assessment and community dementia service.
• Assess service users using a variety of assessment tools, specialised observational and communication skills.

What you’ll need:

• Relevant Professional Qualification in OT

• Current unrestricted registration with HCPC

• Specialist knowledge across a range of procedures and theory

• Professional knowledge acquired through degree/ diploma.

• Significant experience of undertaking Occupational Therapy assessment, formulation, risk assessment and positive risk taking

• Substantial experience in a dementia or mental health setting.

• Must be car driver.
