Summary:
I'm currently helping Equilibrium Healthcare recruit an experienced Occupational Therapist for their Moston Grange site in Manchester. This site provides a service for people with a diagnosis of, but not limited to: Dementia or other associated conditions, alcohol-related problems including Korsakoffs, acquired brain injuries, progressive conditions such as Huntington's and mental health conditions.
Job Duties:
* Act independently in prioritising patient needs to ensure individual patients receive intervention timeously and appropriately.
* Act independently to plan, implement, evaluate, treat and progress patient care, incorporating risk assessment, to maximise functional independence and rehabilitation potential within the specialist area.
* Manage own caseload of patients effectively and efficiently.
* Be responsible for the comprehensive assessment of those patients / clients with a complex presentation, using investigative and analytical skills with guidance of supervisor.
* Contribute to the diagnosis of conditions.
* Work as member of a team to ensure effective communication and delivery of care.
* Maintain patient documentation, records and accurate statistical information to reflect care provided and meet professional standards, including computerised systems.
* Communicate with and make recommendations to, all relevant disciplines of staff to maximise patient care and promote multi-disciplinary working.
* Attend and report to relevant clinical reviews and case conferences ensuring effective communication and co-ordination of patient care.
* Be involved in the development and assessment of the impact of appropriate clinical guidelines ensuring clinical effectiveness to optimise patient care.
Experience:
* Qualification in Occupational Therapy
* Knowledge and application of occupational therapy practice within specialist clinical field of Elderly Crae/Dementia/Mental Health
* HCPC Registered
