Job description


An exciting opportunity is now available for the position of Patient Flow Coordinator within the Discharge to assess team, supporting Discharge to Assess in terms of information gathering and processing transfer of care documents as well as independently supporting patients with discharge planning across University Science Park, West park Hospital and New Cross sites.
Supporting patients into the Discharge to assess pathways. This will be a mixture of Admin, face to face and discharge facilitation
You will work as part of the Discharge to assess team and be visible across the Trust to ensure that systems are in place for the effective flow of patients between departments including, aiming to provide a high quality service and meet the trusts targets. Dealing with relatives, staff from other hospitals, and internal departments across the Trust is a key part of the role.
To lead on facilitating flow of patients through departments that includes but not limited to the University Science Park, New Cross Hospital and West Park Hospital.
The post-holder will participate in the discussions with staff in multiple areas to co ordinate the timely transfer of patients to provide a high quality service and meet Trust targets.
