Personal Adviser (Leaving Care) – Cambridgeshire

£20 to £21.50 per hour

Contract – Full Time

Supporting Care Leavers aged 16-25 as they transition into adulthood

Responsibilities include:

• Direct work and intervention supporting preparation for independence
• Asessment, planning and review (Pathway Plans)
• Working with partner agencies in DWP, Housing, Health and Education to achieve the best outcomes for care leavers
• Deliver group work
