Job description

Role Context and Purpose of the Personal Advisor role

An aspect of the Preparing for Adulthood plus team is to provide practical help and direct support to young people and young adults using the service from age 16 and are in care, or have recently left care (up to the age of 25) or who are unaccompanied asylum seeking children, to become independent and inspire them to set ambitious goals for themselves, so that they:

· Are assisted in making the transition between care and independence

· Have the skills, knowledge and confidence they need to live independently, including to manage their money

· Have appropriate accommodation options

· Access education, training and employment

· Live a healthy lifestyle and access health support when they need it

· Develop and maintain social networks that will sustain them throughout their lives.

What we require from you?

1. Manage a caseload involving the assessment, planning, implementation and evaluation of appropriate action, to ensure that the right resources are deployed to safeguard and promote the welfare of care experienced young peoples

2. To use agreed assessment frameworks in collaboration with the individual to robustly assess their needs and to develop corresponding tailored intervention Pathway Plans setting out clear goals and who is going to provide assistance.

3. To review the Pathway Plans at no less than six monthly intervals, in partnership with the individual.

4.Identify accommodation needs and assist young people in securing appropriate accommodation options. Ensure the educational, training and employment needs of young people are assessed and met.

5. To maintain accurate and up to date case records which comply with departmental policy and procedures, ensuring service users are aware of their right to access information. Maintain accurate records of all work undertaken and produce case reports and correspondence for a variety of audiences including meetings, conferences, panels and other agencies.

6. Build professional credibility within the multi-professional partnership by achieving practice standards and meeting performance targets consistently.

7. Responsible for expenditures from an agreed budget when appropriate. For example, negotiating with care experienced young people’s best value expenditure from Setting up Home Allowance. (SUHA)

Ideally we are looking for Personal Advisor with at least a years experience.

Rates start at £25 P/H dependant on experience and local authority.
