Job description
The post holder will be responsible for working as part of the AHP Discharge Team within Craigavon Area Hospital. The initiative aims to: Reduce inappropriate hospital admissions.Minimise hospital length of stay.Participate in proactive discharge planning alongside the MDT and provide support with complex discharges.Ensure the involvement of patients in the discharge process to improve the patient experience.Identify and establish solutions regarding areas of practice, or organisational development that require improvement.Participate in the monitoring and evaluation of discharge performance.Monitoring the effectiveness of current discharge pathways.Measuring the impact on acute and non-acute services due to admission avoidance and facilitation of discharge. maintaining effective communication between acute, non-acute and community services.working with the Multi-Disciplinary teams in hospital, ICS, Acute Care at Home, Non-Acute Hospitals and other relevant Trust community services.Pro-actively managing and advising senior management regarding pressures on a day to day basis..

Registered with the Health & Care Professional CouncilMember of the Chartered Society of Physiotherapy or Irish Society of Physiotherapy or eligible to apply , Experience of providing postgraduate training to other physiotherapists and the multi-disciplinary team.Experience of delivering / planning group work
