Population Health Management Programme Manager

The purpose of this role is to support implementation of PHM programmes and projects to address health inequalities in Hillingdon and this will entail working across Hillingdon Health Care Partners (HHCP). HHCP is made up of Hillingdon Hospitals NHS Foundation Trust, Central and North West London NHS Foundation Trust (CNWL), H4All, a partnership of voluntary sector health care providers; and Hillingdons GP Federation, which brings together all of Hillingdons GPs working alongside the local authority. The post involves developing an approach that underpins the delivery of national, regional and local initiatives in a coherent, joined up way to target resources and enhance the impact of our programmes on health and care outcomes. The post requires work both autonomously and collaboratively to support system-wide activities.
