Salary: £28,080 to £28,579.20 Annum (Salary based on a 48-hour contract. A higher range is offered to applicants with qualifications in health and social care)

Key duties

  • Providing personal care – including bathing, toileting, dressing and help with feeding
  • Medication administration
  • Assisting and getting involved in residents' daily activities, hobbies or outings
  • Following care plan guidance alongside Company policies and procedures
  • Developing an open, honest, and considerate working relationship with residents
  • Maintaining written daily records
  • Attending and completing training, as required

One day a week that will be assigned to you from management will be the role of Positive Intervention Practitioner. Key duties for this role are:

  • To promote and maintain the positive well-being of the people who live in services provided by Choice Care by assisting in the provision of Positive Behaviour Support Framework for individuals supported by the organisation.
  • To uphold high standards of Positive Behaviour Support in line with policy and practice.
  • To develop and maintain a special interest in recovery-based approaches and support for people with learning disabilities and / or mental health difficulties.
  • Support the workflow and the needs of the assigned Service.
  • As guided by the Clinical Lead and the Senior Practitioner to undertake assessment, formulation work alongside individuals in the context of their history and relationships. This will occur within a broad framework of Positive Behaviour Support and Recovery based thinking.
  • To visit and co-ordinate on going behaviour monitoring of challenging behaviours in relevant CHOICE CARE assigned service.
  • To assist in the development of interventions aimed to reduce the intensity, frequency and potential challenge of behaviours that are challenging to the individual and the service.
  • To work with members of support staff to promote independence and the other outcomes as identified in the personalised Positive Behaviour Support Planners.
  • To offer clearly specified individual and group work to people who receive Choice Care services under the supervision of the Clinical Lead and or the Senior Practitioner.
  • To facilitate the smooth and efficient implementation of the Care Plan systems within the homes.

Where will you work?

Bramerton, in Bray, near Maidenhead, is a large, detached house supporting up to 11 male residents of varying ages, with moderate to severe learning disabilities and associated complex needs. The property is set in extensive gardens with a greenhouse and vegetable patch, tended by the residents, as well as a small summer house. Bramerton has its own Day Activity Coordinator, who develops personalised programmes to support education and life skills, providing a range of engaging and meaningful activities, tailored to individual capabilities. There is a dedicated day services building adjacent to the main house, which provides a base for activities including IT and computing, arts and crafts. The home has transport for regular trips to the cinema, the theatre, bowling, meals out and the nearby Legoland theme park.
