Preparing for Adult Life (PfAL) Practitioner
Adult Social Services
The Preparing For Adult Life (PfAL) Service is responsible for the transition of young people moving in to support provided by Adult Social Services.
We have two vacancies which will involve having a caseload which may be county-wide with an office base to be discussed with the successful applicants.
A key focus of these roles will be to promote independence and support young people to develop least restrictive care packages, greater independence and move into appropriate and local options in a timely way.
It is expected that the successful candidates will develop real expertise on least restrictive care options and tenancy based care packages as an alternative to residential care.
In this role, you will develop real insight into the opportunities in the local care market across Learning Disabilities, Autism, Mental Health and Physical Disability. You will also work effectively with other professionals to achieve these aims across Children’s Services, Health, the Voluntary Sector and Education.
You will have the ability to work with young people with complex needs, as well as their families. You will be able to demonstrate an understanding of the legislative requirements in relation to the Care Act 2014, as well as other legislation including the Children and Families Act 2014.
As part of this role you will be expected to carry out assessments and care planning under the Care Act 2014. Due to the expectations and requirements of this role we would particularly encourage applications from candidates who hold a professional qualification in Social Work.
