We are seeking x2 experienced Programme Leads who are able to offer end-to-end strategic commissioning support throughout the independent living commissioning cycle. They will delivery commissioning, transformation and service improvements activity across a portfolio of adult social care. The services we commission range from transitioning into adulthood to elderly people in nursing homes but these two roles are likely to focus on “support at home” (also known as homecare) and learning disabilities.



You will be innovative and demonstrate leadership acumen. With an understanding of what excellence looks like and the commitment to genuinely shape the future with residents, you will need to build and maintain effective partnerships. An ability to work at pace, dynamic thinking and strong resilience is needed to support us into the next part of our journey.

We want our co-produced Vision for Independent Living to drive everything we do so Independent Living becomes a reality for residents. This means the focus of our work must be about promoting rights, equality and inclusion. Independent Living comes from Disabled people’s ideas and thinking. It is about having full access to all the things we need to live a good life such as jobs, family and friends, housing, leisure, access to transport and enough money. It is also about having the right health services and personal support. That is why we use The Social Model of Disability which is about removing the barriers and discrimination that Disabled people face so we can all be included.



Our approach to commissioning and partnerships must be built on these foundations. It will mean building local capacity, investing in resident-led organisations, creating the conditions for genuine self-directed support, innovation and local services to promote Independent Living. We need to facilitate the best use of resources recognising the fantastic and diverse community capacity here in Hammersmith & Fulham as seen in its vital response to the pandemic.



As Programme Lead for Adult Social Care you will be part of the wider commissioning team charged with transforming the way the commissioning operates so that it can deliver excellence through Compassion and Ruthless Financial Efficiency
