Job summary
The Enhanced health in care homes (EHCH) framework is for primary care networks (PCNs) and providers and commissioners of community health and social care services. It guides the commissioning of and delivery arrangements for health and social care services in collaboration with care homes, so that the entire system works together to provide personalised care, improve outcomes and promote independence for people living in care. It recognises the life course of people who live in a care home and promotes the ethos of living, ageing and dying well.
Sussex Community Foundation Trust have an important part to play in ensuring this framework is successful and alongside our established Care Home Matrons (CHM) an important and exciting element of EHCH is to introduce telemetry opportunities into the care homes both residential and nursing homes. We've completed Phase 1 focusing on Nursing Homes and to commence Phase 2 which will focus primarily on Residential Care Homes.
The Project Co-Ordinator role is integral to the success of the project and would be an exciting challenge for an experienced administrator with excellent key talents such as communication and organisational skills. The role covers trust wide and such the successful candidate's base is negotiable.
The role would suit a Digital/IT forward positive person comfortable with the use of common IT systems. A knowledge of SystmOne would be useful.
