Job overview
This is an exciting opportunity for Band 5 Children's Wellbeing Practitioner to join the Single Point of Access service in West Sussex, based in West Building at our Swandean site, as a 0.5WTE member of the team.
SPOA is designed so that children, young people, their families, carers and professionals can refer easily via a single route. SPoA practitioners will direct and signpost young people to the most appropriate emotional wellbeing and/or mental health services.
Main duties of the job
This is a new post to work alongside SPoA practitioners to agree on the most appropriate care pathways for children and young people referred into the SPoA.
This involves working in partnership with children, young people and their families and the wider professional networks to gather more information in order to make informed decisions on the most appropriate care pathways for young people

Detailed job description and main responsibilities
The SPOA aims to ensure:
  • A more integrated aligned and co-ordinated service is in place
  • More CYP have access to appropriate mental health services at the earliest opportunity through appropriate signposting to services.
  • More CYP have positive experiences of care and support
  • Quicker responses to signposting to the most appropriate services to receive treatment and support is reduced
  • Improved data collection and evaluation of CYP's outcomes

To participate in case discussions and to develop an understanding of available services and evidence based therapeutic intervention to inform decision making of the most appropriate service to signpost young people too.
To work independently with work managed and goals agreed and reviewed through regular supervision.

Qualifications
Essential criteria
  • The minimum required qualification for the Post Graduate Diploma is a 2:1 Degree. The minimum for the Graduate Diploma is A-Levels at grade minimum of ABB, or minimum of BBC (if relevant further training and experience)
  • Proven record of previous academic attainment.
  • Good solid basic literacy and numeracy qualifications at a minimum of level 2
