About the Role:
Dorset Council is seeking a qualified Social Worker to join their Home First Team, dedicated to facilitating efficient and effective hospital discharges. This is an excellent opportunity for a practitioner with a passion for strength-based practice and experience in complex case management including work related to the court of protection.
Day-to-Day Duties:
- Support hospital discharges by coordinating with healthcare providers and community resources.
- Conduct assessments and develop care plans that promote independence and well-being.
- Work collaboratively with individuals, their families, and multidisciplinary teams to ensure smooth transitions from hospital to home.
- Apply a strengths-based approach to support individuals in achieving their personal goals.
- Navigate complex cases, including those involving the Court of Protection, ensuring legal and ethical standards are met.
- Travel within the community to provide support and conduct assessments as needed.
Requirements and Qualifications:
- Qualified Social Worker with current registration.
- Proven experience in the hospital discharge process.
- Strong understanding and application of strengths-based practice.
- Experience or knowledge of working with complex cases and the Court of Protection.
- Excellent communication, assessment, and organizational skills.
- Ability to work independently and as part of a team.
